At the beginning of psychological testing, Dr. Pratt asks parents to gather all of the student's school records. These records give Dr. Pratt needed context to interpret psychological testing data.
Parents gather records that Dr. Pratt needs for psychological testing. When available, Dr. Pratt prefers to have records in a PDF format. Please do not send jpg files. To share files, please do the following:

- Please name records with the date first (e.g., "2023 01 02 IEP Initial").
- Do not include the student's name in the file name.
- Do not use special characters in file names (e.g., < > : " / \ ' ? * ).
Dr. Pratt would like records from all private schools, public schools and school districts where the student has been enrolled.
Private schools vary widely in terms of records they keep. However, Dr. Pratt recommends asking for a copy of all records that they have for the student. Typically, records would include report cards, standardized testing and any accommodations plans.
For students enrolled in a public school, the parent or education rights holder has the right to request the student's cumulative record and special education records. Be sure to request it in writing. You are welcome to copy, paste and modify the list below for your letter.
Sample Letter to Request Public School Records
I am writing regarding my child, a student who is undergoing psychological testing with Dr. Brendan Pratt. I am requesting a copy of all educational records held by the school and school district. I am requesting a copy of the cumulative record, special education records, counseling files and all other records held by the school and school district. That is, I would like all records including, but not limited to, the list below:
- All report cards, progress reports and transcripts for my child.
- All standardized testing results.
- All attendance records, including SARB letters.
- All medical and school health records, health office logs, behavior logs and medication records.
- All notes regarding telephone calls, meetings and other minutes.
- All unsatisfactory reports, warning notices, detentions, suspensions and incident reports.
- All documentation related to SST, CST, 504, IEP or other school meetings and plans.
- All audio and/or visual recordings of school meetings, such as IEPs or 504 meetings.
- All assessment plans, requests and referrals for evaluation, and evaluation reports.
- All testing protocols from assessments completed in relation to an IEP or 504 plan.
- All pages for the print out for every test that was computer scored, including scores and interpretation and intervention suggestions.
- All notes of teachers, psychologists, speech and language therapists, resource specialists and other personnel who have provided services to, evaluated or otherwise have been involved in, or are responsible for, the provision of a free appropriate education for my child.
- All notes and reports of classroom observations of my child by any school or district personnel.
- All correspondence regarding my child to me or any other person, including emails, letters and memos.
- Any other records or documents that identify my child by name.
